Definition

Each time the client is being assessed will be given an Assessment ID and the score the client received for each Outcome Domain will have a unique ScoreID. There are 20 different Outcome Domains across 3 Outcome Type that a client can be scored against. A client can also be assessed for the same domain before and after the session.

Although the variable ‘Assessment Type’ exist with values of ‘Pre’ and ‘Post’ Score, it is not a current indicator whether the client is assessed pre and post. This field was being used prior to August 2018. The determination of pre and post is assessed by a series of algorithm including the Session conducted date, Session ID, the type of Outcome being assessed and Assessment Order.

Despite the complexities of Assessment Order, the Client Assessment table contains Client ID, Attendance ID, Activity ID, DeliveryOrgID and Outlet ID that can be linked to Attendance, Activity, Client and other tables.

Specific Information:

Organisation’s Session Id. This is unique for a particular case within the organisation.

SessionId and CaseId compound must be unique within the upload file.

A client’s name, part of a client’s name, or other identifiable information should not be used as a Session Id under any circumstances.
