Definition
A code that indicates existence of an individualised, dynamic written plan or a support agreement that is developed between family in the program and agency (delivered or resourced by the IFP program) as a result of an assessment process may include;
- Goals and objectives;
- Planned measures/actions;
- Needs assessment (including: health, education, therapeutic social/cultural support);
- Family/relatives/kin contact arrangements;
- Living/accommodation arrangements.
